Millions of Muslims from over 180 countries converge on Makkah and Madinah each year. The scale of that gathering creates conditions highly conducive to the spread of infectious disease: crowds, close physical contact, shared facilities, and the physical demands of the pilgrimage itself.

The Saudi Ministry of Health takes this seriously. There are mandatory vaccination requirements that must be met before a visa will be issued, and additional vaccines strongly recommended to protect you and those around you.

Why Vaccination Matters for Pilgrims

The pilgrimage environment is unlike almost any other. During Hajj, over two million pilgrims gather simultaneously for rituals including Tawaf and the standing at Arafah. Physical proximity is unavoidable. During Umrah, tens of thousands may be present at any given time.

Beyond the rituals, pilgrims share accommodation, transport, food environments, and sanitation facilities. Many travel from countries with endemic diseases rare or absent in the UK. The combination of international mixing, physical exertion, sleep disruption, and unfamiliar food and water can compromise even a healthy immune system.

Pilgrims who cannot demonstrate vaccination compliance may be refused entry. Getting vaccinated at the right time, with the right documentation, is as important as your visa application.

Mandatory Vaccinations

MenACWY (meningococcal meningitis)

This is the single non-negotiable requirement for all Hajj and Umrah pilgrims aged 1 and over. You will not receive a visa without documented proof of vaccination.

Large Hajj gatherings have previously been associated with meningococcal outbreaks. The disease can progress from early symptoms to life-threatening illness within hours. Vaccination is the only reliable protection.

Polio

Required for travellers arriving from countries with ongoing poliovirus transmission, including parts of Pakistan, Afghanistan, Nigeria, and the Horn of Africa. This applies regardless of your UK vaccination history if you have recently been in a high-risk country.

If you are unsure whether this applies to you, our pharmacists will advise. Even if it does not apply, a polio booster is sensible pre-pilgrimage preparation. At Pharmacy M, the DTP vaccine covers polio, diphtheria, and tetanus in a single injection: £50.

Yellow Fever

Required for pilgrims aged 9 and over arriving from countries with a risk of yellow fever transmission, primarily sub-Saharan Africa and parts of South America. Most UK-resident pilgrims will not need this unless they have been in a yellow fever risk country before travelling to Saudi Arabia.

Seasonal influenza

Strongly advised by both Saudi health authorities and UK travel health guidelines. Respiratory illness is the single most common health problem among Hajj pilgrims. Older pilgrims and those with underlying conditions are at particular risk. Get the flu vaccine as close to your departure date as practically possible.

COVID-19

Recommended based on current WHO SAGE guidance. Both NHS and privately administered COVID-19 vaccines are recognised. Speak to our pharmacists if your status needs updating.

Your Pre-Pilgrimage checklist

Work through this at least 6 to 8 weeks before departure.

  1. MenACWY vaccine booked and administered at least 10 days before travel
  2. Certificate obtained, signed, stamped, and stored with your travel documents
  3. Polio status confirmed; DTP booster arranged if needed
  4. Flu vaccine booked
  5. COVID-19 status reviewed and documented
  6. Regular medications reviewed with your pharmacist; sufficient supply packed

Why meningitis is the primary concern

Meningococcal disease is caused by Neisseria meningitidis, which spreads through respiratory droplets and direct contact with nose and throat secretions. In dense gatherings, these transmission routes are almost impossible to avoid.

The disease can cause bacterial meningitis or meningococcal septicaemia. Both can be fatal within 24 hours of first symptoms without prompt treatment.

The MenACWY vaccine covers the four strains most associated with epidemic spread: serogroups A, C, W, and Y. Historical Hajj outbreaks have primarily involved serogroups A and W.

The vaccine is a single injection, well tolerated, and takes effect within 7 to 10 days. Side effects are mild: soreness at the injection site, mild headache, or brief fatigue.

Your official vaccination certificate

The certificate issued after your MenACWY appointment is an official travel health document, not a simple receipt. It must be presented during your visa application and may be checked again at the Saudi border.

At Pharmacy M, your certificate is stamped and signed by our pharmacist and issued on the day of your appointment.

Key facts about your certificate:

  • Valid for 5 years from the date of vaccination
  • Must be issued at least 10 days before arrival in Saudi Arabia
  • The name on the certificate must match your passport exactly. Bring your passport to your appointment
  • Make a scanned digital copy as a backup, but Saudi authorities will require the physical document at the border
  • If you have lost a previous certificate, the safest course is to be revaccinated and obtain a new one

Additional health considerations

Older pilgrims. The pilgrimage involves considerable walking, often in intense heat, with disrupted sleep and emotional intensity. If you have hypertension, diabetes, heart disease, or a respiratory condition, speak to our pharmacists about managing your health during the journey. Pilgrims with chronic conditions. Carry sufficient supplies of all medications for the full duration of your trip, with a buffer for unexpected delays. Carry medications in hand luggage where possible, with clear labelling. Discuss high-temperature storage requirements with our pharmacists if relevant.

Families with children. Children aged 1 and over are covered by the Saudi MenACWY requirement. The vaccine is available at Pharmacy M for children aged 2 and over. Families should ensure all eligible members are vaccinated before departure.

Heat and hygiene. Saudi Arabia during Hajj season can be exceptionally hot. Stay well hydrated, wear light clothing, use sunscreen, and be alert to signs of heat illness. Good hand hygiene reduces the risk of gastrointestinal illness, which is common in any large gathering.

Is the MenACWY vaccine really mandatory? Yes. It is a hard entry requirement. A visa will not be issued without it.
